All new Contractors, Suppliers and Vendors are required to complete and return the forms listed below and attach the requested documentation before your company may be considered an approved vendor.  Payments for services cannot be made for goods and services without the proper documents on file.

  1. W-9 Request for Taxpayer Identification Number and Certification [download W9}

  2. Certificates of Insurance:

  • Certificate of Liability Insurance from your insurance agent with a minimum of $1M in Liability Coverage. This certificate must list MAM + HOA as an additional insured.

  • Certificate of Automobile Insurance from your insurance agent with a minimum of $1M in Liability Coverage. This certificate must list MAM + HOA as an additional insured.

  • Certificate of Workers’ Compensation insurance (Or in the case of a Sole Proprietor, the Sole Proprietor/Independent Contractor workers compensation indemnity agreement must be completed and signed. Or in the case of a LLC, an exclusion form with a State of Michigan stamp must be completed).
